PCOS AND OVULATION INDUCTION . A JOURNEY TO CONCEPTION WiTH IVF-WORLD

• Couple married for 4 to 5 years. wanting to stat a family.
• wife experienced unexpected changes in her body like gaining weight, hypertension and pre diabetes.
• diagnosed with PCOS And High AMH values .
• Anovulation cycles - no natural follicles at her period cycle.
• ovulation induction done to promote ovualation through oral medications(letrozole)and low dose of gonadotrophin injections to prevent from hyper stimulation of ovaries.
• injectable gonadrotrophins - daily injections of FSH (follicle stimulating hormone to promote development of mature follicles to increase chances of in vitro fertilisation.
• couple had 19 follicles, 15 embryos were formed.
• transfer was skipped for the present cycle to help the ovaries return to normal due to some hyper stimulation . it is always advisable in case of hyper stimulation ( more follicles forming )
• meanwhile estradiol and progesterone given to maintain healthy endometrium lining and healthy uterus to enhance chances of conception .
• five embryos cultured for 3 days , embryo transfer planned on day 3.
• 3 embryo transferred on day 3, wife conceived twins out of which one turned out to be blighted ovum ( a type of miscarriage when the embryo stops growing ), because of which the wife had one healthy on going pregnancy.
• the remaining embryos were frozen and preserved , if the couple wanted to have another pregnancy in the future

this case shows that patients with PCOS can be hyper responders ( 19 follicles retrieved) and PCOS can manifest itself and influence fertility treatments .
